Job Posting: Transition Case Manager 

POSITION SUMMARY:  Responsible for member enrollment and internal transition. Monitors quality and of member placement and ensures all member transitions met the best practice standards of Teaching Family Association, applicable state and federal regulations and agency quality assurance standards.

Transition Duties:

  • Respond to referrals within 24 hours.
  • Conduct member observation and complete member social history
  • Coordinate Meet and Greets with agency personnel and third-party stakeholders
  • Provide or schedule transportation for a member to attend meet and greets
  • Coordinate discharge meeting
  • Coordinate and facilitate agency transition meeting
  • Follow-up and track home modification status for new member enrollee(s)
  • Create member profile in PHS
  • Establish member home and office book  
  • Schedule mover and/or assist with member move
  • Complete Day of Transition process and welcome packets
  • Monitor and track referral trends and publish by the 5th of each month
  • Facilitate and Coordinate agency respite request

QUALIFICATIONS:
Bachelor’s degree with at least 1 year of experience in a related field. 
Valid Driver's License.
Able to complete online and classroom training to include

CPR and First Aid
CPI
Pass a background check
Pass a drug screen

Pay rate: $ 35,000.00- $38,000.00 per year.

Pay Comments: depending on experience, education & license
